understand that 2013 Summer Youth Dance Company requires a six (6) week commitment to learning, rehearsing and performing a dance program with Xalaat Africa and DanceCircus from Monday June 24 to Saturday August 3, 2013. I agree to participate in all classes, rehearsals, special events and traveling performances to the best of my ability (except as indicated by parent/guardian on next page).

Tuition for 6 week 2013 Summer Youth Dance Company: $425.00*
One half tuition and Costume fee is due at time of registration (see audition notice below).
Full amount due June 25, 2013, first day of class. Payment plans available
INCLUDES $25.00 FEE FOR COSTUMES

Interested in Work-Study Scholarship Auditions? Reserve you space in the Work-Study Scholarship Audition class on Thursday June 20, 2012 beginning at 9:30am and ending at 11:00am no later than 24 hours in advance. Auditionees will need completed application forms A (student Registration) and B (parent/guardian permission) to participate in audition. Full and partial Scholarships are available. Contact DanceCircus dance@dancecircus.org or 414-277-8151 to register.

NOTES:
Application forms A (student Registration) and B (parent/guardian permission), with Parent/Guardian signature, is required for all students ON June 24, 2013 THE FIRST DAY OF CLASSES. Student lacking signed permission forms will not be allowed to participate until forms are submitted.

Students will receive information on dress code for classes upon receipt of full or partial tuition or work-study scholarship audition.
